Objective: Establish common metrics and goals, build definitional frameworks, and coordinate data sharing agreements to ensure continued alignment on progress and accountability. 

Key Strategies: 

• Population-Level: Update results statement, targets, and progress indicators to drive actionable strategies through the Results-Based Accountability framework. 
• Program-Level: Through Hartford Data Hub, enhance data sharing capacity of HOYC members to address key research questions and promote opportunities for improved services coordination. 
• Individual-Level: Pilot common intake, referral, and case management system approach within CNSDI and consider lessons to apply city-wide.
